Updated charts to include dolutegravir's European product label

The interaction charts have been updated to include data from the European Summary of Product Characteristics for dolutegravir which was issued in January 2014.  As a result of recommendations in the European label, azole antifungals and nelfinavir have been changed from amber (potential interaction) to green (no clinically significant interaction expected). In light of this, we have reviewed all dolutegravir amber interactions and have changed others from amber to green. The full list of updated interactions is as follows:
Chloramphenicol, Clarithromycin, Dasatanib, Diltiazem, Echinacea, Erthyromycin, Everolimus, Fluconazole, Garlic, Griseofulvin, Imatinib, Itraconazole, Ketoconazole, Lapatanib, Miconazole, Mifepristone, Nefazodone, Nicardipine, Posaconazole, Quercetin, Temsirolimus, Verapamil, Voriconazole

All versions of the charts (web, app, printable) have been updated, along with the Treatment Selectors. In addition, the printable versions of the charts now include four new drugs (ivabradine, probenecid, sitagliptin, tolterodine) that were added earlier this year to the web and app charts.
