Summary:
Coadministration with estradiol as hormone replacement therapy (HRT) has not been studied but based on metabolism and clearance a clinically significant interaction is unlikely. Estradiol is metabolized by CYP3A4, CYP1A2 and is glucuronidated. Tenofovir-DF does not interact with these metabolic pathways. When estradiol was administered as part of feminizing hormone therapy to 20 HIV-infected transgender women receiving efavirenz and emtricitabine/tenofovir-DF, estradiol AUC, Cmax and C24 decreased by 28%, 19% and 36%, respectively, likely due to enzyme induction by efavirenz. There was no significant change in tenofovir pharmacokinetics.
